All visitors to Tanzania must complete the Tanzania Health Surveillance Form, which requests for basic personal information, health information, and travel-related information. The purpose of the Tanzania Health Surveillance Form is to limit the spread of coronavirus and protect visitors.

Every tourist must complete and submit the form within 24 hours of entering Tanzania. In the event of a probable coronavirus infection transmission, Tanzanian authorities may approach them based on their provided contact information. Quarantine may be immediately established for a passenger infected with the virus and its spread may be prevented as a result.


What Steps do I need to follow to fill out the Tanzanian health Surveillance Form?

In order to obtain certification of completing the Tanzanian Health Form, each candidate must complete three basic steps. You must have an electronic device with a stable internet connection in order to complete the online form. The following steps are:

  1. In order to pay the processing fee, a variety of online payment options are available to visitors.
  2. To verify the applicant’s email address, the Health Form for Tanzania will be sent to the individual’s email account.
  3. It is also strongly suggested to print the file and carry a hard copy of it throughout the trip.

What information do I need for an online application of Surveillance Form?

  • You will also be asked a few health-related questions, be sure to answer them.


All overseas visitors to Tanzania must complete a Zanzibar Health Form in addition to a primary Health Form. Individuals who plan to visit Zanzibar must complete a separate Health Form. Both forms must be finished within 24 hours of the scheduled arrival.

All entrants must provide a COVID-19 test that is negative. The test must be administered within 96 hours of the scheduled departure time. Authorities in Tanzania accept either RT-PCR or NAATs.

When travelling from countries with high coronavirus infection rates, travellers from these countries will be subject to extra testing at their own cost before filling the Surveillance Form.

The quarantine period will last 14 days while they await a negative test result. Countries subject to testing on entry are available online.
